Retrieval
Fee and
Expedite
$25.00 per file optional
This
includes
normal
copy
fee, the
expedite
fee is
to hand
deliver
your
request
to
Kaiser
and
speed up
the
retrieval
processes.
In
addition,
T¡¯s Copy
Service
will
follow
up on
your
request
once a
week by
phone.
(Note,
however,
T¡¯s Copy
Service
cannot
guarantee
this to
work on
all
requests
since we
do not
control
Kaiser¡¯s
procedures.)
If you
choose
not to
use this
service,
then you
must
submit
your own
request
to
Kaiser
with a
check
for
$15.00.
T¡¯s
Copy
Service
must
also be
on the
authorization
sheet
per
example
below to
be able
to copy
your
records.
Kaiser
locations
out of
Santa
Clara
County
will
have
additional
pricing,
please
call for
information.
Note:
Kaiser
facilities
located
outside
of San
Jose
will
have a
travel
fee of
.40
cents
per
mile.
This may
be
waived
if two
or more
files
are
ordered
